Please help...Submit button gives error

I am using the dreamweaver
The website is called www.hoian-okinawa.com
The problem is on the customer service page
The form gives an error when you submit it

I think it's because you're sending a post request to a .html file instead of a .php file?

Your "action" is blank. You need to send the information through a script of some sort like a PHP contact script.

yep I guess I'll start from there
so basically I just use that form and it should work
